EWP Design Request Form Instructions | When submitting plans, it’s important for us to have the following information to provide the most efficient and accurate design/ quote. Please provide the information listed below, check the boxes that apply. Dealer | Dealer Rep | Project Name | Project Due Date |

Floor System-General Overview Full layout Quick quote (Select this option if you are needing a budget number. No layout will be provided) Beam Selection Complete Beam Package: Include all necessary beams in the design where possible. Flush Beams in Floors only: Other - Please Specify Will we be doing the design per plan? Yes No (If the design will be per the plan, and you specify whether we are quoting a complete beam package or just flush beams, that’s all we need. However, if the design won’t follow the plan, we will require additional details.) EX. | If changes, please outline them clearly. For example, the plan may call for 2x10 floor joists and a stick-framed roof, but the builder might want to consider I-joists for the floors and roof trusses. • For any changes, we need to know the plan for each floor. Is it a 2x floor system, I-joist, or floor trusses? The same applies to the roof—stick-framed or trussed. See selections below. Are you open to Value Engineering? Yes No EX. | Changing footer placement, joist direction, on-center spacing If yes, we will review the plans and suggest value-engineered solutions. • All recommendations will be discussed and confirmed before we begin the design, either in a documented conversation or via marked-up plans.
